You can access support from Creative Connections OT with the below funding options:

 

NDIS:

We accept NDIS funding through:

Plan Managed (The invoice is sent from Creative Connections OT to your plan manager)

Self-Managed (The invoice is sent to you and processed through your self-managed account).

Medicare: 

You may be eligible for a rebate on 5 sessions per calendar year under the Chronic Disease Management Plan (CDMP). Consult with your GP for further information and to organise this.

Private Consults: 

Consults are paid for privately, a GP referral is not required.
